What is Enstilar?
Enstilar is a prescription medication that is classified as a topical corticosteroid (betamethasone) and vitamin D analog (calcipotriene).
What is Enstilar used for?
It is used for the treatment of plaque psoriasis. The foam formulation is indicated for patients 12 years of age and up.
How does it work?
The calcipotriol component induces differentiation and suppresses proliferation of keratinocytes, which reverses the abnormal keratinocyte changes in psoriasis. The betamethasone component has anti-inflammatory, antipruritic, and vasoconstrictive effects.
How to apply Enstilar?
Wash your hands before applying Enstilar.
Do not apply to the axillae, face, groin, or on skin with preexisting skin atrophy at the affected site.
This medication is not for intravaginal, ophthalmic, or oral use.
Do not use any occlusive dressing after application unless directed by your doctor.
What dosages are available?
Enstilar is only available at an external foam agent (betamethasone dipropionate 0.0064% and calcipotriene 0.005%; 60 g).
What is the recommended dosage?
Apply the spray foam to the affected area once daily.
What is the maximum dosage?
Do not exceed 60g every 4 days.
What if I miss a dose?
Take the medication as soon as possible if you have missed a dose. If it is almost time for your next dose, skip the missed dose and return back to your regular schedule. Never double the doses.
What should you know before you take Enstilar?
The foam formulation contains flammable propellants. Use should be avoided around flame, fire, and smoking during or right after the medication application.
Adrenal suppression may occur, particularly in younger patients or patients receiving high doses for a long period of time, on altered skin barrier, under occlusion, or use over a large surface area, and patients with hepatic dysfunction. If HPA axis suppression occurs, stopping Enstilar, decreasing application frequency, or using a less potent corticosteroid should be attempted. HPA axis suppression is usually reversible.
Allergic contact dermatitis can occur. Further evaluation may be necessary.
Caution in patients with known or suspected any calcium metabolism disorders.
There have been case reports of Kaposi sarcoma in patients with prolonged treatment of corticosteroids. If this occurs, Enstilar therapy should be stopped.
Avoid contact with eyes as posterior subcapsular cataracts and glaucoma have been reported with topical corticosteroids. Monitor for visual symptoms.
Local adverse side effects can occur, such as burning, atrophy, allergic contact dermatitis, striae, telangiectasis, itching, dryness, irritation, acneiform eruptions, folliculitis, perioral dermatitis, hypopigmentation, secondary infections, and miliaria. The risk is increased with prolonged use, occlusive use, or use of higher potency corticosteroids.
Absorption of topical corticosteroids can cause glycosuria, hyperglycemia, or Cushing’s syndrome.
Skin infections should be treated with appropriate antifungal or antibacterial agents. If the infection does not go away, stop Enstilar until the infection resolves.
Avoid excessive exposure of the treated/affected skin from natural or artificial lighting due to photosensitivity.
Systemic absorption of topical corticosteroids may cause hyperglycemia or unmasking of latent diabetes mellitus.
It is unknown whether Enstilar is excreted into breast milk.
