Rexulti (Brexpiprazole) is prescription medication that is classified as an atypical antipsychotic.

Uses

What is Rexulti used for? 

It is used as an add-on therapy to antidepressants for the treatment of major depressive disorder (MDD) as well as for the treatment of schizophrenia.

How does it work?

It exerts its antidepressant effects by acting as a partial agonist at serotonin 5-HT-1A and dopamine D2 receptors, and as an antagonist at serotonin 5-HT-2A receptors.

Take Rexulti (Brexpiprazole) as directed by your doctor. Taking it at the same time each day can help to avoid forgetting doses.

What dosages are available?

Rexulti (Brexpiprazole) is available as brand 0.25mg, 0.5mg, 1mg, 2mg, 3mg, and 4mg tablets.

What is the recommended dosage?

Generally it is dosed 1mg once daily but take as directed by your doctor.

What is the maximum dosage?

The maximum recommended dosage is 4mg/day.

What if I miss a dose?

Warnings & Precautions

What should you know before you take Rexulti (Brexpiprazole)?

There is an increased risk of mortality in elderly patients with dementia related psychosis when taking antipsychotic drugs (class related).

There is a chance that this medication can increase the risk of suicidal thoughts and behaviors in patients who are 24 and younger in short term studies. Rexulti (Brexpiprazole) has not been studied in pediatric patients.

Increased risk for cerebrovascular accidents in elderly patients with dementia-related psychosis in placebo controlled trials with 2nd generation antipsychotic medications. (class related)

Patients with existing diabetes should exercise caution while taking this medication as it can cause hyperglycemia and in some extreme cases can be associated with ketoacidosis.

Atypical antipsychotics can exacerbate dyslipidemia in some patients so it is important to have a baseline lipid profile completed as well as routine monitoring there after.

A post marketing analysis reports that some patients experience increased urges, significantly in regards to gambling, and an inability to manage and control these urges. Make sure you talk to your doctor if you start to experience compulsive urges.

Patients with that are CYP2D6 poor metabolizers should inform their doctor as this prompts an alteration in the recommended dose.

Rexulti (Brexpiprazole) is mainly metabolized by CYP3A4 and CYP2D6, as a result, medications that are strong inhibitors/inducers of these enzymes should be identified and dosing should be adjusted accordingly. Some examples are listed below.

  • Strong CYP3A4 Inhibitors: itraconazole, clarithromycin, ketoconazole
  • Strong CYP2D6 Inhibitors: paroxetine, fluoxetine, quinidine
  • Strong CYP3A4 Inducers: rifampin, St. John’s wort

Does Rexulti (Brexpiprazole) cause low white blood cell count?

Neutropenia/leukopenia have been reported with other antipsychotic agents within the same class as Rexulti (Brexpiprazole), if you notice signs and symptoms of an infection make sure and notify your provider as soon as possible. If you develop a white count <1000 cells/mm3 your provider will likely discontinue the medication until it returns to normal.

What is Rexulti (Brexpiprazole) approved for?

Per the manufacturer and FDA, Rexulti (Brexpiprazole) is approved as an adjunctive treatment for major depressive disorder and for schizophrenia.

Does Rexulti (Brexpiprazole) make you hungry?

During the clinical trials 3% of patients reported having an increased appetite.

Does Rexulti (Brexpiprazole) help with anxiety?

In the clinical trials the study participants for MDD had MDD with or without anxiety and their improvement was based on the change in their MADRS score. This scoring system does have an anxiety component. Rexulti (Brexpiprazole) is not indicated for anxiety. Approximetly 3% of the study population reported anxiety as an adverse event.

Is Rexulti (Brexpiprazole) addictive?

No, Rexulti (Brexpiprazole) has little habit forming potential, in the studies Rexulti (Brexpiprazole) did not demonstrate any withdrawal signs upon discontinuation, talk to your doctor first before deciding to discontinue Rexulti (Brexpiprazole).
