About Eliquis (Apixaban)

What is Eliquis?

Eliquis is a prescription medication that is classified as a factor Xa inhibitor, direct oral anticoagulant (DOAC).

Uses

What is Eliquis used for? 

It is used for the treatment of deep vein thrombosis, nonvalvular atrial fibrillation, pulmonary embolism, and postoperative venous thromboprophylaxis following knee or hip replacement surgery.

How does it work?

It binds to the active site of bound and free factor Xa which results in decreased thrombin generation and thrombus formation.

Directions

How to take Eliquis?

Take your Eliquis tablet by mouth twice daily with or without meals as prescribed by your doctor.

After knee/hip replacement, you should take your Eliquis 12 to 24 hours after your surgical procedure.

If you are unable to swallow your tablet whole, you may crush the tablet and suspend it in apple juice, D5W, or water (administer immediately).

What dosages are available?

Eliquis is only available as a brand as individual tablets (2.5mg, 5mg) or a tablet therapy pack (5mg; 74 tablets).

What is the recommended dosage?

The following are dosing recommendations for the various indications: 

  • For stroke reduction in patients with afib: 5mg twice daily 
  • DVT prophylaxis after knee or hip replacement: 2.5mg twice daily 
  • Treatment of DVT/PE: 10mg twice a day for 7 days then 5mg twice a day thereafter.

What is the maximum dosage?

5mg twice daily is the max maintenance dose (1mg twice daily for loading regimen for DVT/PE treatment)

What if I miss a dose?

Take the medication as soon as possible if you have missed a dose. If it is almost time for your next dose, skip the missed dose and return back to your regular schedule. Never double the doses. Taking more than the prescribed dose will significantly increase your risk of bleeding.

Warnings & Precautions

What should you know before you take Eliquis? 

Eliquis is not recommended in patients with triple-positive antiphospholipid syndrome (APS).

An increased risk of stroke was observed in patients transitioning from Eliquis to warfarin when used to prevent strokes in those with nonvalvular atrial fibrillation.

Caution with moderate hepatic dysfunction. No dose adjustments are provided due to limited clinical experience with this patient population. Use with severe hepatic dysfunction (Child-Pugh class C) is not recommended.

Caution with renal dysfunction. Some experts recommend avoiding use of Eliquis in severe renal dysfunction (CrCl < 25 mL/min, including dialysis) until more robust data is available.

Eliquis is not recommended in patients with prosthetic heart valves as it has not been studied in this patient population.

There is limited data with the use of Eliquis in pregnancy. If planning to become pregnant, you should speak to your doctor as there is a clinical significant risk of uterine bleeding.

Use of Eliquis is not recommended in women who are breastfeeding.

U.S. Boxed Warning:

  • Discontinuation:
    • Premature discontinuation of any oral anticoagulant increases the risk of a blood clot (thrombotic event).
    • If Eliquis is stopped for any other reason besides pathological bleeding, another anticoagulant should be considered to provide coverage.
  • Spinal/Epidural Hematoma:
    • Spinal or epidural hematomas may occur in those treated with Eliquis who are undergoing spinal puncture or receiving neuraxial anesthesia, which can result in long-term or permanent paralysis.
    • Patients should be frequently monitored for signs/symptoms of any neurological impairment. If neurological compromise is detected, urgent treatment is required.
    • Weigh the risks vs benefits of Eliquis therapy.

What are the side effects of Eliquis?

Some common side effects include:

  • Hemorrhage

Interactions

Does this medication interact with other drugs, foods, or alcohol?

Avoid use with other anticoagulants, hemin, strong CYP3A4 inducers, mifepristone, omacetaxine, St. John’s Wort, urokinase, and vorapaxar (Risk Category X).

Caution with grapefruit juice as it may increase levels/effects Eliquis. Patients should be cautioned/advised that starting grapefruit juice while on Eliquis can increase the risk of bleeding.

Storage

How should this medication be stored?

Eliquis should be stored between 20°C – 25°C (68°F-77°F). Excursions are allowed to be between 15°C – 30°C (59°F-86°F).

Pharmacist Tips

  • When taking this medication, you will notice that you may bleed (e.g., from the gums after brushing or nose) and/or bruise more easily and that it may take longer for the bleeding to stop. Get help immediately if:
    • The bleeding does not stop or is severe.
    • You are vomiting up blood or if your vomit looks like coffee grounds.
    • Pink/red/brown urine or red/black tarry stools
    • Unexpected bleeding, unexpected pain, swelling, or joint pain.
    • Any headaches, dizziness, or weakness.
  • Do not stop your medication without speaking to your doctor that prescribed the Eliquis for you.
  • Grapefruit juice may increase the risk of bleeding with Eliquis, so monitor for any signs of bleeding.
