How does it work?

Triiodothyronine (T3) and levothyroxine (T4) diffuse into the cell nucleus and bind to thyroid receptor proteins attached to DNA which affects DNA transcription and protein synthesis.

How do I take Synthroid (Levothyroxine)?

Take Synthroid (Levothyroxine) as directed by your physician, on an empty stomach 30 minutes to 1 hour prior to breakfast.

What dosages are available?

Synthroid (Levothyroxine) is available as brand 25mcg, 50mcg, 75mcg, 88 cg, 100mcg, 112mcg, 125mcg, 137mcg, 150mcg, 175mcg, 200mcg, and 300mcg tablets.

What is the recommended dosage?

Generally it is dosed generally 1.6 mcg/kg once daily but take as directed by your doctor.

What if I miss a dose?

What should you know before you take Synthroid (Levothyroxine)?

Synthroid (Levothyroxine) can put you at increased risk of heart related adverse reactions. If you have a pre-existing heart condition make sure your doctor is aware before you start taking Synthroid (Levothyroxine). If you experience heart related symptoms (increased heart rate, blood pressure, abnormal heart rhythm, chest pain, shortness of breath) while taking Synthroid (Levothyroxine) you should seek medical attention. 

Synthroid (Levothyroxine) has what is referred to as a narrow therapeutic index. This means that there is a fine line between undertreating and overtreating your condition. This also means that it is imperative to follow all instructions given by your physician as failing to do so can either continue to precipitate your ongoing condition or produce potentially dangerous adverse events. 

Patients with existing diabetes can expect Synthroid (Levothyroxine) to worsen their ability to control sugar levels. Most patients will require more insulin to regulate their blood sugar after starting Synthroid (Levothyroxine). It is very important for these patients to regularly check their blood sugar after starting Synthroid (Levothyroxine) and after each potential subsequent change in their dose of Synthroid (Levothyroxine) to reduce risks of hyperglycemia (or hypoglycemia if there is a dose reduction). 

If you are on a supratherapeutic (too high) dose for an extended period of time it could put you at risk for decreased bone mineral density. Particularly in post menopausal women, you should ensure you are having your thyroid levels checked routinely to ensure that your dose requirements have not changed.

What foods should be avoided when taking Synthroid (Levothyroxine)?

Certain foods can affect the absorption of Synthroid (Levothyroxine). Soybean flour, cottonseed meal, walnuts, and dietary fiber may bind to and decrease the absorption of Synthroid (Levothyroxine). Additionally grapefruit juice may delay time of absorption and affect how much gets into the bloodstream. Avoiding these is ideal but ensuring adequate separation of these will help prevent interactions from occurring.

What are the most common side effects of Synthroid (Levothyroxine)?

The most common side effects of Synthroid (Levothyroxine) are related to supratherapeutic doses (too high). arrhythmias, myocardial infarction, dyspnea muscle spasm, headache, nervousness, irritability, insomnia, tremors are some of the symptoms you may experience. 

Can you drink coffee right after taking Synthroid (Levothyroxine)?

All food and drink, aside from water, should be separated from Synthroid (Levothyroxine) by at least 30 minutes to 1 hour as these can affect the absorption of the medication. 

Do you increase Synthroid (Levothyroxine) if TSH is high?

Generally, if your thyroid stimulating hormone (TSH) is elevated beyond a certain range, your physician will increase your Synthroid (Levothyroxine) dose. TSH is produced by your body in response to low levels of circulating thyroid hormone but given that your body has a dysfunction/inability to produce more thyroid hormone the TSH can accumulate given there is not an adequate response made by your body so additional supplementation is required. 

How long can you go without thyroid medication?

It is not advised to go without your thyroid medication as this may result in you reverting to symptoms of hypothyroidism. It will vary from person to person but the half-life (the time it takes for half of the medication to be cleared from your body) of Synthroid (Levothyroxine) is about 9 to 10 days.
