Bone fractures: Some clinical trials have shown an increased incidence of bone fractures with other SGLT2 inhibitors. Meta-analyses of Jardiance (Empagliflozin), however, have not shown an increased risk of fracture. 

The data is conflicting with the risk of lower amputations with SGLT2 inhibitor use. Canagliflozin use was associated with almost a two-fold increased risk of lower limb amputations versus placebo in the CANVAS and CANVAS-R trial. However, the FDA has removed the Boxed Warning regarding the risk of amputation with the use of canagliflozin from the prescribing information due to new data from 3 clinical trials. Patients should be counseled about the importance of preventative foot care.  

Renal dysfunction:

• The U.S. manufacturer does not recommend initiating Jardiance (Empagliflozin) if the eGFR < 45 mL/min/1.73m2 and to stop Jariance if it is persistently below this threshold. 
• Per the manufacturer’s label, use is contraindicated with (eGFR < 30 mL/minute/1.73m2, however, some experts continue to use Jardiance (Empagliflozin) off label at a dose of 10 mg once daily for the treatment of diabetic kidney disease.
