What is Alvesco? 

Alvesco is a prescription medication that is classified as an inhaled corticosteroid.

Uses

What is Alvesco used for?

It is used as a prophylactic maintenance therapy for the treatment of asthma in patients 12 years of age and up. 

How does it work?

The exact mechanism of how this medication treats asthma is unknown but as a corticosteroid, it reduces inflammation which is the typical cause of asthma exacerbations.

Directions

How do I use Alvesco (Ciclesonide)?

Step 1: If your Alvesco is new or it has been 10 or more consecutive days since use, you will need to prime your inhaler. Priming consists of releasing 3 puffs away from the face after removing the plastic cap and ensuring that the indicator display window is before the “60” inhalation mark. 

Step 2: The actuator should be held upright, between your thumb, middle finger, and forefinger, with the mouthpiece facing you.

Step 3: The canister should be firmly placed in the mouthpiece with each use. Do not shake your Alvesco before use. Breathe out fully as comfortably as you can. Close your lips around the mouthpiece and keep your tongue below it. 

Step 4: Press down on the center of the dose indicator using your finger while breathing in slowly and deeply. Be sure to press down fully on the canister until it stops moving in the actuator. Hold your breath for 10 seconds or as long as comfortable, once you have finished breathing in.

Step 5: Remove your finger completely off the center of the dose indicator. Remove your inhaler from your mouth while gently breathing out.

Step 6: Put the cap back on the mouthpiece. This is to keep the mouthpiece clean.

Step 7: Rinse out your mouth with water and spit it out. Do not swallow.

What dosages are available?

Alvesco is available as an aerosol inhalation solution in 80 mcg/actuation (6.1 g) and 160 mcg/actuation (6.1 g). Alvesco 6.1 g canisters contain 60 inhalations. Dosing varies and is based on previous asthma therapy and severity.

What is the recommended dosage?

 It is typically dosed as an 80 mcg inhalation twice daily but take as directed by your doctor.  

What is the maximum dosage?

The maximum recommended dosage is 320 mcg inhalation twice daily.

What if you miss a dose?

If a dose of this medicine is missed, skip the missed dose and go back to your regular schedule. Do not double the doses.

Warnings & Precautions

What should you know before you take Alvesco?

Adrenal suppression can occur, particularly in young children or those receiving high doses for a prolonged period of time, which can lead to adrenal crisis. The corticosteroid should be tapered slowly and carefully if withdrawing or discontinuing the medication. 

Long-term use of corticosteroids can cause immunosuppression, increasing the risk of infection, prolong/exacerbate viral infections, or decrease response to a vaccine. Avoid if possible, use in patients with ocular herpes, active or quiescent respiratory tuberculosis, untreated bacterial, viral, parasitic, or fungal systemic infections. 

Long-term use of an inhaled corticosteroid can cause a decrease in bone mineral density. Use with caution in those with major risk factors for a low bone mineral count. 

Use with caution in those with ocular diseases, such as cataracts and/or glaucoma. Long-term use of corticosteroids can cause blurred vision, increased intraocular pressure, cataracts, and glaucoma.

Due to reports of systemic corticosteroid withdrawal, a gradual tapering of dose may be necessary prior to discontinuation of therapy.

What are the side effects of Alvesco?

The most common side effects of Alvesco include:

  • Headache 
  • Nasopharyngitis 
  • Sinusitis 
  • Oral candidiasis 
  • Pharyngolaryngeal pain 
  • Upper respiratory infection
  • Arthralgia
  • Nasal congestion
  • Pain in extremity and back pain

Interactions

Does this medication interact with other drugs, foods, or alcohol?

Corticosteroids may decrease the antineoplastic effect of Aldesleukin and combination use should be avoided (Category X).

Inhaled corticosteroids may increase the hyponatremic effect of Desmopressin and combination use should be avoided (Category X).

Agents used to treat airway disease can enhance the toxic/adverse effect of Loxapine. This is specific to the Adasuve brand of loxapine which is an inhaled formulation and does not apply to the non-inhaled formulations. Combination use should be avoided (Category X).

Inhaled corticosteroids may decrease the diagnostic effect of Cosyntropin and therapy should be monitored (Category C).

Strong CYP3A4 inhibitors may increase the active metabolite(s) of Ciclesonide (Inhalation) and therapy should be monitored (Category C).

Smoked tobacco may decrease the therapeutic effect of inhaled corticosteroids and therapy should be monitored (Category C).

Storage

How should this medication be stored?

Alvesco should be stored at room temperature between 15°C to 30°C (59°F to 86°C) away from direct light and heat. Do not freeze your inhaler. 

Do not puncture your Alvesco canister. Do not store your Alvesco canister near heat or flame. Do not freeze your Alvesco inhaler. Keep your Alvesco clean and dry at all times.

When your medication is expired or no longer needed, safely throw away the medication. 

Keep out of reach of children.

Pharmacist Tips

  • Your inhaler must be primed before the first use (releasing 3 puffs away from the face) or when the inhaler has not been used for 10 consecutive days or more.
  •  Do not shake the inhaler before use.
  • Rinse mouth with water and spit out after each use to decrease risk of oral candidiasis from occurring.
  • Do not wash your inhaler or place it in water. You should clean your inhaler weekly with a dry cloth or tissue.
  • Be sure to discard after the “discard by” date or when the dose indicator display window reads “0”, even if the canister is not empty.
