Xeljanz (Tofacitinib) is a prescription medication that is classified as an janus kinase (JAK) inhibitor.

What is Xeljanz used for?

 It is used to treat rheumatoid arthritis (RA), psoriatic arthritis, and ulcerative colitis (UC). 

How does it work?

Xeljanz is thought to disrupt Janus kinase (JAK) enzyme activity. This JAK enzyme activates components in the body that initiate the immune response. When the immune response is reduced by Xeljanz then the signs and symptoms of arthritis and ulcerative colitis are reduced.

How do I take Xeljanz (Tofacitinib)?

Xeljanz (Tofacitinib) should be taken as directed by your physician. The immediate release tablets are dosed twice daily and the extended release tablets are dosed once daily. They can be taken without regard for food. 

What dosages are available?

Xeljanz (Tofacitinib) is available as brand 5mg and 10mg immediate release tablets. It is also available in brand 11mg extended release tablets.

What is the recommended dosage?

What if I miss a dose?

This medication can reduce your immune system’s ability to fight off infections. It is not advised that you take it during a serious active infection. 

Although there was not a discernible difference between patients on Xeljanz (Tofacitinib) and placebo with regard to gastrointestinal perforations, patients at increased risk for these perforations should use caution while taking this medication. 

Patients on Xeljanz (Tofacitinib) should have their lymphocyte counts monitored initially and every 3 months thereafter while one therapy. 

Avoid live vaccines while on Xeljanz (Tofacitinib). 

What are the side effects of Xeljanz (Tofacitinib)?

Some common (≥2%) side effects for patients with RA include:

Xeljanz (Tofacitinib) is metabolized by CYP3A4. Medications that inhibit or induce this enzyme should be used with caution. Additionally, medications that are immunosuppressive in nature should be avoided where possible. 

How should this medication be stored?

Xeljanz (Tofacitinib) should be stored in the original packaging at room temperature (68°F to 77°F)

• Xeljanz (Tofacitinib) may inhibit your ability to fight infections, if you believe you have an active infection contact your physician for a proper evaluation. 
• If you are of child bearing potential, talk with your doctor prior to taking Xeljanz (Tofacitinib) as it may have fertility implications that may or may not be permanent.
