What is Synjardy?

Synjardy is a prescription medication that is classified as a combination product containing a sodium-glucose co-transporter 2 (SGLT-2) inhibitor (empagliflozin) and a biguanide (metformin).

Uses

What is Synjardy used for?

It is used in adjunct with diet and exercise to achieve glycemic control in patients with type 2 diabetes that were unresponsive to empagliflozin or metformin monotherapy.

How does it work? 

The empagliflozin component inhibits SGLT2 which reduces the renal reabsorption of glucose resulting in increased glucose excretion. The metformin component increases peripheral glucose uptake and utilization to exert its glucose lowering effects.

Directions

How do I take Synjardy?  

Synjardy should be taken as directed by your doctor. It should be dosed with meals to lessen the chance of stomach upset. While taking this medication, check your blood sugar regularly; your doctor will also monitor your blood sugar through blood tests periodically. You may also need to take other diabetes medications along with Synjardy, refer to your doctor for your specific diabetes management plan.

What dosages are available?

Synjardy is available by brand-only 5 mg/500 mg, 5 mg/1000 mg, 12.5 mg/500 mg, and 12.5 mg/1000 mg tablets.

What is the recommended dosage?

The specific dosage formulation of Synjardy depends on how controlled your diabetes was on your previous diabetes medications but generally it is dosed twice daily with meals.

Warnings & Precautions

What should you know before you take Synjardy?

Avoid excessive alcohol use and use in patients with hepatic impairment or in hypoxic states as it may result in lactic acidosis

This medication may result in hypotension. Patients should be assessed for appropriate volume status prior to therapy.

Renal function should be monitored during therapy

Discontinue Synjardy temporarily if patient is to undergo radiologic studies requiring iodinated contrast materials or if surgery requires restriction of food and fluid intake

The risk for hypoglycemia is increased if Synjardy is combined with insulin products; the dose of insulin may need to be lowered

Risk for genital mycotic infections, urinary tract infections, and vitamin B-12 deficiency

Synjardy may increase your LDL levels; your doctor will monitor and treat as appropriate

Currently there are no clinical studies to establish conclusive evidence that Synjardy has macrovascular risk reduction

  • Symptoms of lactic acidosis include: feeling very weak/tired, unusual muscle pain, trouble sleeping, cold sensations in your arms and legs, and slow heartbeat. If you experience any of these symptoms, immediately seek medical attention.
  • There is a higher risk for experiencing lactic acidosis if you have renal impairment, liver impairment, have congestive heart failure, binge-drink alcohol, are dehydrated, have a history of heart attack, or are over the age of 80.
  • Symptoms of hypoglycemia include: fatigue, sweating, fast heartbeat, hunger, irritability, and confusion. If you experience any of these symptoms, check your blood sugar.
